A CBCT scan is not just a better X-ray. It is a three-dimensional surgical map. For the implant dentist, reading it correctly is the difference between a predictable outcome and an intraoperative surprise.
The Five-Step CBCT Reading Protocol
Step 1 — Bone Height
Measure available bone height from the alveolar crest to the critical anatomical landmark: the inferior alveolar canal (mandible) or the floor of the maxillary sinus (posterior maxilla). You need a minimum of 1–2mm safety margin below the landmark. Available height minus safety margin = maximum implant length.
Step 2 — Bone Width
Measure at 3mm below the crest (the implant placement depth for most systems). You need minimum 6mm width for a standard 3.5mm implant with 1.25mm bone on each side. Below 5mm, consider a narrow-diameter implant or ridge augmentation.
Step 3 — Bone Density (Misch Classification)
- D1: Dense cortical bone — anterior mandible. Drill at full speed, full irrigation.
- D2: Thick cortical + coarse trabecular — anterior maxilla, posterior mandible. Standard protocol.
- D3: Thin cortical + fine trabecular — posterior maxilla. Underdrill by 0.5mm, consider wider implant.
- D4: Almost no cortical layer — posterior maxilla. Condensing osteotomes, platform switch, longer healing time.
Step 4 — Root Proximity
Identify adjacent root positions and angulations. Minimum 1.5mm clearance between implant surface and adjacent root is required to prevent vascular disruption and root resorption.
Step 5 — Anatomical Variants
Check for: bifid inferior alveolar canal, high sinus floor septae (Underwood's septae), submandibular fossa concavity, anterior loop of the mental nerve. These variants change your surgical approach and must be identified before you make the first incision.
The Clinical Habit
Never rely on 2D periapical films alone for implant planning. A CBCT costs the patient ₹2,000–3,500 in India. A complication costs infinitely more. Build CBCT prescription into your implant workup as a non-negotiable step.
